Directions for First Visit Paperwork
The following forms should be downloaded and printed, completed and brought
with you to your initial visit. If you have completed this paperwork before
your visit you may arrive five minutes before your scheduled time (if
not, please arrive 30 minutes before your appointment). Each form can
be downloaded in Word or Adobe Acrobat.
Patient
Policies - Please read our office policies form. This will answer
most of your questions regarding how our office functions. Please be sure
to sign both the general policies and the privacy policies in order to
protect your personal medical information. Bring these signed documents
with you to your first appointment.
Personal
Medical History - Please complete this form to provide us your
medical history and bring it with you to your first appointment.
Review
of Symptoms - Circle any items that apply regarding your current
symptoms or problems. This completed form should be brought with you to
your first appointment.

Information - This is a two-page form that has your personal
and insurance information (address, phone number etc. so that we can contact
you). This form must be brought to your first appointment.
